ADAC appointments are scheduled with our physician on the second and fourth Tuesday of each month at 711 Troy Schenectady Road, Latham, NY.

An ADAC assessment, which includes the clinic visit, involves a three-step evaluation process:

1. A home visit and assessment is completed by the ADAC social worker prior to the clinic visit.

2. During the clinic visit, a physician who specializes in the needs of the elderly will complete an examination and may order particular laboratory tests and diagnostic studies. The ADAC physician will review lab findings previously gathered by the person's primary care physician and may suggest a referral to a specialist.

3. A family conference follows the clinic to allow the ADAC team to present recommendations for supportive care services based on the diagnosis and design a possible plan of care that fits the current needs of the person and their family members with linkages to appropriate care services, The ADAC team also provides suggestions for future care options to address changing needs over the course of the disease.

What is the cost for ADAC services?
Fees for ADAC services are determined on a sliding scale depending on income. Medicare, Medicaid, and some private insurance providers may cover physician services and costs related to diagnostic testing.
