Private Care Management
Eddy Home Care offers private professional care management services to help people make informed decisions for themselves or a family member -- usually a parent -- about the complex issues associated with aging or living with disabilities. Our experts can guide individuals seeking solutions to health, housing, safety and asset management concerns; and connect people to the community resources they need to remain at home.
This program helps individuals of all ages, including senior adults, individuals with disabilities, children with chronic diseases, and family members or others who have assumed responsibility for the care of someone in need. Private care managers can:
- assess client psycho-social status and financial considerations
- identify appropriate resources available to accommodate individual requirements and expectations
- coordinate comprehensive individualized care plans and services for individuals with special needs who want to remain independent and at home
- maintain contact with the client's immediate family members (including those at a distance) and other responsible parties who wish to be involved in the care planning process
- can monitor and evaluate the effectiveness of client's care plan when responsible family members are temporarily unable to supervise care
- guide family members faced with the sometimes overwhelming responsibilities and emotional isolation of caregiving, or loss of a loved one
- support individual clients coping with depressions associated with disability
This service offers support and promotes community-based independence for clients for as long as it is comfortable, and when necessary. It also aids in the transition to adult or nursing home placement, and Eddy care managers may also assist attorneys and trust officers who are advising clients and their families about long-term planning for future wellness and well-being.
Eddy Home Care also offers the Eddy Resident Service Advisor Program (RSAP), a unique service that helps seniors who live in selected senior housing communities in the Capital Region to find and access the services they need to remain independent in their own homes.
